A Jazz de Havilland Dash 8-400, registration C-GJZF performing test flight QK-7190 from Toronto,ON to Toronto,ON (Canada) with 4 crew (2 pilots, 2 maintenance engineers), was on approach to Toronto's runway 24L when the crew extended the landing gear, however, did not receive a green indication for the left main gear. The crew went around, worked the related checklists but were unable to resolve the unsafe indication. The crew declared PAN PAN and prepared for a landing with the unsafe indication, shut down the left hand engine and landed safely on runway 15L about 65 minutes after aborting the first approach.
The Canadian TSB reported the aircraft had performed a test flight after a spoiler cable had been replaced. The TSB wrote: "Upon inspection on the runway, maintenance discovered a tool jammed in the left landing gear, preventing it from fully extending. Subsequent investigation determined that the tool found was an extension magnetic tool, not under the operator's control, and lacking any identification markings. The operator conducted research into prior maintenance in the area but found no documented reason for the tool to be located there."
