A KLM Boeing 737-800, registration PH-BCD performing flight
KL-1696 from Lisbon (Portugal) to Amsterdam (Netherlands) with 126 passengers and 11 crew, planned for a departure from Lisbon's runway 21 at intersection S1, however, the flight planner did not havy any data for that intersection departure. Taxiing out the crew was instructed to line up runway 21 at intersection U5 and commenced takeoff. The main gear became airborne 284 meters short of the runway end, the aircraft crossed the runway end between 45 and 70 feet AGL. The aircraft continued to Amsterdam for a safe landing without further incident.
Portugal's GPIAA rated the occurrence a serious incident reporting, they were notified about the occurrence on April 29th 2021 only. The GPIAA reported neither captain nor first officer detected any issue or lack of takeoff performance. Data suggested however, that the takeoff performance had been computed for a full length (or intersection S4) departure (3705 meters/12,160 feet), the actual takeoff distance available however was 1395 meters/4,580 feet less leaving 2310 meters/7,580 feet available.
The GPIAA stated: "When using the U5 intersection with parameters of the S4 position, the take-off is developed with 1395m less than planned. This implies that the speed and altitude of the aircraft at the end of the runway may not be adequate for an All Engines Operative (AEO) condition and pose an unacceptable risk in the event of a One Engine Inoperative (OEI) condition."
The investigation is ongoing.
