A Sky Airlines Airbus A320-200N, registration CC-AZE performing flight
H2-5142 from Arequipa to Lima (Peru), was descending towards Lima when the crew reported a hydraulic failure. The aircraft continued for a safe landing on Lima's runway 15 and stopped on the runway due to lack of nose wheel steering.
Peru's AIB reported the fluid from the yellow hydraulic system was completely lost causing also the failure of the nose wheel steering. In addition the tread of tyre #2 had detached causing damage to the lower and aft surface of the left wing in addition to fracturing and bending the hydraulic line located in the left main gear department. The occurrence was rated a serious incident and is being investigated.
Peru's AIB released their final report in Spanish only (Editorial note: to serve the purpose of global prevention of the repeat of causes leading to an occurrence an additional timely release of all occurrence reports in the only world spanning aviation language English would be necessary, a Spanish only release does not achieve this purpose as set by ICAO annex 13 and just forces many aviators to waste much more time and effort each in trying to understand the circumstances leading to the occurrence. Aviators operating internationally are required to read/speak English besides their local language, investigators need to be able to read/write/speak English to communicate with their counterparts all around the globe).
The report concludes the probable cause of the serious incident was:
Total loss of hydraulic fluid from the "Y" main hydraulic system due to a crack in the return tube in the wheel well of the left main gear, that was caused by impact of rubber debris from the tread of tyre #2. The rubber detached from the tread of tyre #2 during takeoff from Arequipa prompting the crew to work the related actions associated with the alarm messages and subsequently an emergency landing in Lima without inconvenience.
The AIB summarized the sequence of events stating the crew monitored and verified the progressive loss of hydraulic fluid from the yellow main hydraulic system, informed Lima South Control requesting priority and requesting a tow after landing. The aircraft landed safely and was towed to the apron.
