A Fly Amelia Embraer ERJ-145, registration F-HYOG performing flight
8R-1217 from Rodez to Paris Orly (France) with 39 passengers and 3 crew, landed on Paris Orly's runway 25 at 19:19L (17:19Z) however went off the runway. The aircraft came to a stop with all gear on soft ground. There were no injuries.
The BEA have dispatched 4 investigators on site.
On Oct 27th 2022 the BEA reported: "During landing, at night, the airplane left the runway longitudinally.", rated the damage to the aircraft minor and the occurrence a serious incident.
On Dec 5th 2023 the BEA released their final report in French only (stating the English version will be released in due time, the AVH will post the analysis when the English report becomes available) concluding the probable causes of the serious incident were:
- Lack of sharing of doubts and concerns by each flight crew member causing absence of a "common project"
- break down of crew cooperation during descent and approach in particular failure to respect the difference of roles of pilot flying and pilot monitoring
- evolution towards an autocratic cockpit with a tendency of the captain overstepping his role as pilot monitoring and the first officer as pilot flying accepting this development without seeking to assert himself
- absence of any attempt to re-capture the expected synergy despite the common CRM deterioration
- incomplete management of errors and threats (TEM) in a stormy evolving situation, in particular failure to identify possible runway limitations, downwind and crossind conditions in a thunderstorm environment prone to windshears and showers
- the crew's inappriate management of trajectory and energy leading them to intercept the glideslope from above with an aircraft that was too fast
- continuation of the approach despite most of the stabilization criteria not met and a TAWS "terrain too low" alert that was triggered
- continued landing despite the long landing
