A Cargojet Airways Boeing 757-200 freighter, registration C-FGKJ performing flight W8-1891 from Hamilton,ON (Canada) to Havana (Cuba) with 2 passengers and 3 crew, was climbing out of Hamilton when the crew advised ATC of flaps problems and requested to return to Hamilton, 45 minutes holding to burn off fuel were needed. The crew declared PAN PAN and landed safely back on Hamilton's runway 30 at a higher than normal speed (about 197 knots over ground). The aircraft slowed safely and stopped on the runway, emergency services checked the hot brakes. The aircraft was subsequently towed off the runway.

Canada's TSB reported the flaps were locked out 1 degrees, the trailing outboard edge was extended slightly more than the inboard trailing edge. Maintenance found the torque tube bearing assembly between inboard and outboard flaps had failed, the bearing assembly was replaced.
