A Caribbean Airlines Boeing 737-8 MAX, registration 9Y-CAL performing flight
BW-607 from Toronto,ON (Canada) to Georgetown (Guyana), was cleared for takeoff from runway 15L by tower and began to taxi into position and commence takeoff.
A Flair Airlines Boeing 737-8 MAX, registration C-FLDX performing flight F8-149 from Halifax,NS to Toronto,ON (Canada), had safely landed in Toronto and was cleared to cross runway 15L by ground while 9Y-CAL just began their takeoff roll.
The tower controller recognized the loss of separation on the runway and instructed
BW-607 to reject takeoff (at low speed).
The Canadian TSB reported the controller subsequently explained
BW-607 had not been cleared for takeoff despite the clearance issued earlier. After F8-149 was clear of the runway,
BW-607 was cleared for takeoff again. F8-149 had been crossing the runway at taxiway T about 9200 feet down the runway, the aircraft could not see each other due to the geography of runway 15L, which slopes upward from each end. The TSB rated the occurrence a reportable incident "loss of separation" and a Class 5 occurrence.
